We are happy to file the forms necessary so you can receive the full benefits of your coverage, however we cannot guarantee any estimated coverage.

You are responsible for unpaid balance, deductible and copayment.

Any deductible and/or copayment is due at the time of service.

In the event your insurance does not pay as estimated, you are responsible for the balance due.

For those patients without dental insurance, our policy is that the full amount is due at the time of treatment.
